By scheduling your appointment, you are verifying that:

You will communicate any further inquiries with your esthetician.

You do not hold ARETE THERAPEUTICS LLC or LOVE BUTTON SPA LLC responsible for any of your conditions that were present, but undisclosed at the time of your service, which may affect the treatment performed on the day of your appointment.

You understand the home care instructions given by your esthetician, and you are willing to follow the recommendations that may minimize or eliminate possible negative reactions.

You understand that these homecare instructions are deemed as suggestions and advice, not diagnoses or prescriptions.

You are fully aware of the risks and hazards associated with hair removal/waxing services, facial services, chemical peel services, and/or all body treatment services offered at LOVE BUTTON SPA LLC.

You are fully aware of your voluntary participation and assume full responsibility for any risks of loss, property damage, or personal injury that may be sustained by you or owned by you as a result of any service being performed, whether caused by releases or otherwise.

You have not had any COVID-19 symptoms the past 14 days. You acknowledge that COVID-19 is a contagious virus and voluntarily assume the risk that you may be exposed to or infected by COVID-19 by receiving any services at LOVE BUTTON SPA LLC.

You hereby release, waive, discharge and covenant not to sue ARETE THERAPEUTICS LLC and/or LOVE BUTTON SPA LLC from any and all liability, claims, demands, actions and causes of action arising out of or related to any loss, damage, injury, including death, that may be sustained by you or any property belonging to you whether caused by the negligence of the releases, or otherwise while participating in waxing, facial services, and/or any other services you request to be performed on these premises where the activity is being conducted.
